Below is a SAMPLE Affidavit, this will need to be notarized by a notary republic in your county in order for it to be effective..

If you would like either an ID Card, Affidavit or both, I will provide both the image file for the ID Card and either an editable affidavit or if you need, you can send me your information and I can create them for you. (free of charge)

I can also print and mail you a PVC card in the mail if you like, for this however, I will need to charge for both shipping in the card, I think $10 would be a fair price.

Information I will need if you want me to create both the ID Card and or affidavit.

First and last name, DOB, Height, an ID Number, you can use your SSN or just make up something with at least 9 digits and I will need a photo of your face, make sure it’s fairly clear

For the affidavit, I need your first and last name, County and State you reside and Address or PO Box.

Once you receive the completed document from me, you will need to get the affidavit notarized by a notary republic in your county, usually there are notary’s available at the County Clerk’s office, Post office and some banks although a bank may charge you a small fee.
